The clinicians shown here represent the shared team; their inclusion does not mean that every clinician will work with every client. Before admission, ask who will lead your care, which professionals are expected to be involved, and how questions or changes will be communicated.

How care is coordinated

Assessment brings together your concerns, treatment history, medication, relevant physical health information, and personal goals. With appropriate consent, existing clinicians may contribute information that helps the team understand what has already been tried and what needs continuity.

During a stay, individual therapy and other agreed clinical input should work toward a coherent set of goals. Ask how progress is reviewed, who is your main point of contact, and how your plan can change. Family participation and information sharing are discussed with you and depend on consent and the circumstances. See our approach and guidance for families.

Planning for discharge should clarify who provides ongoing care, what information is transferred, medication arrangements where relevant, and the practical steps for returning home. Continuing care explains the next stage; actual follow-up services and fees are agreed individually.

Admissions and clinical decisions have different roles

Jil Moore, Client Relations Director, and Cynthia Nakhle, Admissions Manager, are part of the shared admissions team. They help explain the process, residential options, fees, and information required for clinical review. These admissions roles are distinct from clinical assessment and treatment.

Admission depends on clinical approval and availability. An inquiry is not a diagnosis or a promise of admission. Some needs require emergency care, medical stabilization, or withdrawal management elsewhere before COGNIFUL can be considered. Read how admission works or professional referral guidance.

Questions to ask before a stay

  • Who will lead my care, and which clinicians are expected to be involved?
  • How will my current treatment and medication be reviewed?
  • What clinical support is included, and what may require separate arrangements?
  • How will my existing clinician or family participate, with my consent?
  • Who will coordinate the transition home?
